Why are my periods so heavy?

There are many causes of abnormal uterine bleeding. If your periods are heavy and irregular most likely you are not ovulating regularly. This (anovulation) could expose the inner ling of uterus to estrogen for prolonged periods of time, causing endometrial hyperplasia, which is a risk factor for endometrial cancer. Other causes of abnormal uterine bleeding in a nonpregnant woman are perimenopause, polyps, and fibroids (benign tumors of the uterus). Treatment for abnormal uterine bleeding depends on its cause, but ranges from contraceptive pills to endometrial ablation, polypectomy, myomectomy or hysterectomy. Do I need hormone replacement therapy?

In the past, hormone replacement therapy was the primary therapy for the prevention of osteoporosis. Numerous studies have been performed evaluating the benefits and risks of hormone replacement therapy. The most recent large study (WHI) has shown a slightly increased risk in breast cancer as well as a small increased risk of clots in veins. Hormone replacement therapy is no longer first-line in the prevention or treatment of osteoporosis. However, it is still quite useful in the management of bothersome hot flashes. Currently, osteoporosis prevention and treatment is managed with bisphosponates (Actonel, Fosomax) or E-receptor modulators (Evista). Do I have PMS?

Premenstrual syndrome or PMS is currently referred to as premenstrual dysphoric disorder or PMDD. Signs of PMDD are various symptoms which interfere with your quality of life, occurring just before a menstrual period and dissappearing within the first few days after the onset of a menstrual period. Such symptoms include but are not limited to the following:

  • Feeling sad or tearful
  • Irritability
  • Difficulty concentrating
  • Change in appetite
  • Breast tenderness
  • Bloating
  • Weight gain
  • Headache
